A signed sample description sheet must accompany submitted samples. We also request that the completed form be submitted to us by email. To download, right-click on the image on the right and select “Save Target As…”

This form can be completed in Excel. Guidance notes for completing the form are included within the file.‌

When completing the form please do not copy and paste into any of the fields from programs other than Microsoft Excel (otherwise this may cause importing issues with our database).

If you require a Purchase Order number to be stated on the invoice, please ensure that it is sent to us before samples are submitted.
